You'd need to find a provider that is willing to give you her real name and address on an invoice, otherwise I wouldn't think your insurance company will process it (assuming they look into it before reimbursement). If she gives you an invoice with a fake/working name and address to protect her identity, and your insurance company does look into it, at best you'll be denied and at worst be accused of submitting a fraudulent claim. Insurance companies are serious about digging into claims to see if there's any way they can not pay out. Cool idea, but I wouldn’t mess with this.
I've developed some back issues that my doctor says would benefit from periodic massages. I like the idea, but quite honestly, it has been a very long time since I've gotten a massage without a fun finish! This may be a stupid question, but....I wonder if any of my fellow board members know of a massage therapist who is set up to provide a professional therapeutic massage, then bill me, so I can submit to insurance or bill my insurance directly....and who will also provide a sensual finish (billed separately, of course!)? While I have never had a "session" with her, Andrea Adams is a licensed massage therapist. If you're willing to travel to Woodbridge.
She bills herself as "99% GFE" which basically means she does everything but FS. But I have heard great things about her massage skills and she's a legit massage professional.
I don't know how she would work out insurance issues but you can reach out to her and find out if she would do that.

Go to a fully legitimate therapeutic massage specialist and have the massage your insurance will cover. Then go see you favorite happy ending therapist for that.
I suspect finding one that is able to provide the invoice for the legal therapy services for insurance that is also willing to provide the additional services may be a bit of a challenge. Moreover, if you've not consider it, should something go wrong you may find yourself confronted with insurance fraud charges. Might not be something that bites you even if you did get caught in a bust but you never know.
Thanks for your note. Your suggestion of keeping the two separate makes sense. After I posted I thought more about the insurance part! I’m all about the challenge of a HE from a CMT and leaving the insurance out of the picture!
In three decades, I have met only two certified massage therapists (CMTs) in the DMV who were also providers. I think you would need a CMT for insurance claims. Unfortunately, one is definitely inactive and is studying to become a nurse (she is a friend of mine). The other, Megan Erotic may also be inactive; she has one year-old review, all of the others are pre-pandemic.

Good points….and agree not to mess with this. Maybe the route is to find a nice looking, professional CMT with a legit business who will cave under my charm and good looks and offers extras “under the table”😀
