I have to very strongly agree with what most of Rachel posted, and also add a few thoughts of disagreement to friend sserviceman, loves...
To say "don't count on any specific answer to this question from 'any' provider because the real answer is 'whatever you really want and will pay for"....
....That may certainly be true across a wide variety of providers and services, but...
i must take a bit of issue with that, as it seems to be a very harshly cold, critical, and judgementally blanket view of all providers, and stated in such a way that cannot possibly be honestly or accurately qualified, as no one here, could have ever possibly been with everyone else out there, nor know what, really in fact, lies between the ears, or within the heart, of any truly good provider, that actually MAY legitimately care about this kind of question/issue.
I would agree with Rachel, 1000% , that to treat EVERY client like a VIP, regardless of donation or class of service issue, unless something SPECIAL is specifically requested otherwise by that person themselves... or offered as "truly extra" by a truly good provider, then maybe that should be the honorable rule across the board without exception.
in direct response to the initial question: If that provider has a very legitimate reason for listing in that way - and regularly lists different classes of services, and which would only consider providing even MORE absolutely exceptional services than what they usually offer to share - something really, truly extra, and they are already an excellent provider, i could see the sense in that.
If part of what serviceman stated is true, and more of the norm for listings like that, then that really does seem kind of sad....
Should someone wish to spend (considerably) more time than would be normal, or have a special request such as fetish outside the "norm", or a long distance outcall or extended travel and companionship, then i believe it is entirely legitimate to class that into a different category of gift exchange, and be utterly up front about it, with that client, beforehand...
....and, even then, it should not change one SINGLE whit of how that provider goes about doing things if they really care about :
a) The quality of truly personal service they offer, without exception, to EVERYONE,
b.) has the both ability and enough genuine love and respect not only for what they do (but every potential client) to effectively carry off the above, and not do it in a "mechanical and emotionally detached session" and,..
c.) technique is similar to a "cold read done by psychic fortune tellers....when they...find out what their customers are really looking to hear" - -
....as most so called "psychics", are fakes and frauds to begin with, this differs considerably from a real flesh and blood TS female that is providing tangible, real, physical, mental, emotional, and intellectual intimacy, and i dont believe is a fair comparison across the board to ALL providers...
I have never changed my rates, nor my own ways of practicing genuine and extremely caring connections, unless special circumstances, or ONLY special requests, as described above, demanded it, or...
the client was experienced enough to know the differences, and had no issue with changes based on what they wanted, in total mutual agreement...
In fact, i dont mind losing a good number of potential seekers in my own screening, because there are things i simply will NOT do, or cannot relate to and still remain genuinely passionate...
...and if that is ever the case, i will either do my best to stretch my own ability to learn and grow (as long as the client knows that is the case and we are mutually agreed with being open and patient to one another), or, be up front about me not being interested in that kind of thing, whatever it may be, and politely excuse myself to recommend they keep looking elsewhere.
A good provider has to maintain her own standards as well, to protect every aspect of her life and practices, especially if working or living in a highly discretionary area/region, and should reserve the right to do so....
One often even MUST do so, especially if ever increasing a rate for specific services will immediately increase the screening out of less desirable types, and connecting with those that are not sketchy, or represent some form of compromise at a critical level of quality and genuine engagement.
Just an honest thought, straight from the heart.... Utterly No offense intended. Not everyone is the same, and not everyone should be painted with the same broad brush strokes...
Most Respectfully, always,

The AngelaGirl
