Please let me know if this post comes across as offensive or rude.  

But, I've recently contemplated getting into the hobby but haven't fully gone through with it. So far my experiences have been at FBSM places but never with a full-service provider.  

I see countless providers offering BBBJs, if not BBFS but in these cases, how does one work through the fear of STDs, specifically herpes?
Given that the providers (high-end or otherwise) offer BBBJ, would the risk of transmission not be significantly higher?

in the odds between BBBJ and BBFS, so you should treat them separately because the risk is not even proportional.  As Rafi says, most providers will be happy to use a cover for BBFS or BBBJ.  While I dabbled in BBFS many years ago, since 2019, I have always used a cover.  However, I have never used a cover for BBBJ, never caught anything from a provider, and I have seen over 1000 providers since I started in P4P in 2007.  

 
Each monger has to choose their own comfort level with these risks.  I have caught a few STD's from providers when I was doing BBFS years ago, but I just put myself on the bench until the meds are finished.  I'm single, but if I had a SO, I would not have taken some of the risks that I have.  If the risks make you anxious, try stamp collecting.  You never lick anything that can hurt you.

I offer non-GFE services for those who prefer to keep it safer. When I first started working as an adult entertainer I would give hand jobs with a glove and covered blow jobs only. I have a standing appointment in the Sexually Transmitted disease clinic of an accredited teaching hospital. I go every two-months for full screening and medication review.

A game changer for me has been an antiviral that I receive by intramuscular injection at this appointment. It lasts 2 months. There are also antibiotics which, taken shortly after sex, will significantly lower your chances of contracting a bacterial STD.

A vital, but sometimes overlooked preventative step is taking a vigorous shower after your tryst.

I remain STD free.

Silicone lube application even if it a small dab drastically reduces chances of catching some thing..
Warning , it just REDUCES...does NOT eliminate or  avoids the problem.
Correctly  and repeatedly (do drying out) applied silicone lube forms a thin layer akin to a condom.

Sorry to break it to you, but there is a risk anytime any activity happens. But you know what? I would trust that the provider I meet is cleaner than a civilian girl I meet at a bar. Provider is much more likely to get tested frequently. How many civilians even get tested? When was the last time you got tested?

The responses on here are frightening. A shower and vigorous scrub will help?!? Lube reduces your chance of catching something !?!? Jesus - this just makes me think going bare is much, much more dangerous than I would normally think if the other customers are approaching sexual health like this. These kind of 'tips' are debunked in 7th grade health class.

Think of it like this - if you don't wear a condom during sex, your chances for catching something increases. If you're hooking up with someone and you're not monogamous, you're chances increase as every new partner (for both participants) increases the chance of catching something. Now consider what that means for a provider who sees, what, 2-8 customers a day. Those chances skyrocket.  

Every discussion like this is basically a group of guys trying to convince themselves it's ok to go bare. We all know better. You'll eventually catch something...you just gotta hope it's cure-able and not herpes. Herpes, btw, can be spread via blowjobs and you don't even need to see signs of an outbreak for it to spread (look it up on CDC's site). And don't think all most these providers are on prohibitively expensive PrEP (2-3k/month w.o insurance). HIV is a possibility.

Long story short don't try to convince yourself you'll be fine. If you're OK with the risk, that's one thing. But it's a legitimate and real risk. And this isn't a knock against the providers - I just don't trust horny mongering dudes enough to believe they're testing at the rate they should.

i second everything just said ..  have i rationalized going bare wayyyyyy too many times, yes ... but i didn't do it blindly, i just tried to use good judgement and hoped for the best ... of course i didn't go bare with some KGirl with 248 reviews ,  it stuns me that guys are fine with that.

That is, don't "work through the fear".  
Instead, do the research (in decent medical sources, not here!!).  
Then if the fear persists, simply don't fuck.

If the fear is abated by knowledge, behave as required to keep the risk within your tolerance.  
For example, choose your fuck partners from those with lower chance of STDs.

And remember to calibrate the risks against the other risks in your life, such as
 - driving to an appointment
 - driving to the grocery store
 - eating pre-packaged produce, candy,  
 - drinking alcohol
 - breathing maskless (flu, Covid, hantavirus, pneumonoconiosis, measles, ... ... )
 - social interaction (conflict, marital conflict, gang suspicion ...)
and so on and so forth.  Each of these has a risk and a benefit.  The tradeoffs are yours to make.

Always a risk.....remember what you are involving yourself in...having said that and in no particular order:

1.  Avoid Ghetto Girls

To play the game bare, both the provider and client should be on prep and doxy. The client should also ask for recent test results and be willing to provide them as well.
