The second provider responded today, scheduling a date with me and apologized for the delay. Still no idea why the provider was too rude to follow up last month, but I'm no longer worried this is a pattern.
I apologize for posting under an alias, but I’m doing so for reasons that will be obvious once you've read this.
In the last couple of months I've had two well-reviewed providers who regularly post on the DC ad board respond enthusiastically to my email inquiries, only to fall into radio silence once I've emailed them my screening information. In many years of hobbying with over two dozen reviews and many White List referrals, I have NEVER done anything that would make a provider afraid to see me. Accordingly, I’m becoming concerned that my name may have unfairly found its way to some sort of hobbyist “blacklist,” or that one of my references has mistaken me for someone else.
I’m looking for advice on any way to find out if I've been blacklisted, and if so anything I can do to correct or address that so that I’m not unfairly frozen out of the hobby. Thanks in advance for your thoughts. Please reply on this forum or PM me.

Some people do flake, but you know, this is not widely known, a lot of email does not actually go through -- like up to 20% of all email! You might have got blown off -- or the ISP may not have ever delivered your message. What a bummer that would be! And when that happens, the receiver is completely unaware they missed a message. So, there is always the ol' reliable phone call
Yes Duchess is right. Who knows why that happens but it does. And then some emails get delivered into spam. I rarely check my spam folders. Maybe once a week or every other week.
