I've seen a number of threads on here lately about bareback type providers, either ones who say they will do it, or those who just plain do it during a session without discussing it beforehand.

I don't doubt anyone who reports that, though I've never had any experiences along those lines.

But what I have seen very commonly is providers who use lubricants that shouldn't be used with condoms. Pretty much any lubricant that contains oil - including mineral oil, hand lotion, etc, not just vaseline - should not be used with a condom since it could weaken it. But in my experience a large percentage of providers try to use lubricants that aren't explicitly condom safe.  I would be sure to watch for this and make sure that they aren't just using condoms, but are using lubricants of an appropriate type.

I had a session years ago with a TS escort that used hand lotion as lubricant. I knew it wasn't condom safe; but in the heat of passion, I went along with it. Luckily, the condom didn't fail. After the session, I realized how stupid I had been. Now, I always bring my own water based lube with me, along with several condoms. One of the tricks of the trade for some escorts is to tell you they just ran out of condoms; so anal sex is not an option.

I too had a similar "wardrobe malfunction" involving massage oil. It was with a GG provider who I had become very comfortable with and this one time the condom failed at shall we say was a very inconvenient time. I didn't even know it had happened but she sure did.
We actually both went and got tested two days later and to our relief there was no further action needed but I've never had such a tense 48 hours as the time between. Definetly a wake up call.
Stay safe my friends...
