Sounds like a blackmail operation.

I've been on SA dates with more than a hundred girls but I've never seen this. Have you?

 
Her ninth message to me was a request to fill out an online form. "Kindly fill out this form to arrange a date with me and I'll contact you promptly," plus a link to the form at Typeform.com.

 
The form asked for my legal name, age, email, phone, desired date, desired time and duration, "verification links," professional title and company name, and a "brief introduction of [me]."

 
I replied to her message by saying, "A form?" Her reply: "Yes! Kindly fill it out if you'd like to arrange a date with me."

 
She's a 19yo 7, mixed (Latina?), major midwestern city, been on the site 3 months, literate but brief narrative.

 
The form reminds me of some of the pro verification sites.

 
I won't reply to her.

It's hard to believe she's not a pro trying to drum up more business on SA.  But she obviously does not understand the very different protocols.  The typical way we vet each other in the sugar bowl is to meet and greet, and see if we both like the vibe.  Filling out a form like the one you mentioned is strictly an escort tool, not something I've ever encountered in the Bowl.  

And probably has an agency (or pimp?) that runs that online form.  

 
Now there is nothing wrong with a pro looking to sugar date either as a personal desire or to grow her bookings on the DL.   I'm pretty sure you would have seen that confirmed when she came back with an hourly rate, rather than a per-date allowance. The question about "desired duration" is the tip off, if the form itself is not.  

 
I have yet to see a POT use a form in the Los Angeles area, but I've certainly (and currently) dated SB's who had a pro book of business.  And I have booked some pros that I ended up sugar dating as well.  Often if you can find an IG account, you will get a better vibe of her. It's interesting that the actual personal info items requested on that form are the same data I always give to my new SB's before we have our 1st BCD date.  Builds trust and confidence.  Note that I am blackmail proof: no wife/SO, retired, friends know I date younger, hotter women.
