I googled "provider blacklist" and found several right off the bat.

If it is a certain blacklist you are very screwed. They well never take your name down even at the provider who blacklisted request. All they can do is add an addendum to the blacklist. The site shows right up on Google and uses real information. It is a national blacklist so it is in every area. I will not share info on this site as I hope it dies from lack of clicks, but know that is unlikely.

I if I was sneaking around on my wife and my name showed up on that site, I would find a good divorce lawyer and put him on retainer as when your wife/boss/friend/child/other Google's your name it will come up. Maybe you can legally change your name? People have tried to sue them with no luck.

There are many other blacklist sites as well, some national, and some are local. Can't speak for those but on the larges public one you are screwed. If you really want to take a look at that site try Google it pops right up.
