You extend your penis through the opening and then urinate. A woman’s flush-surfaced urethral orifice would make such a design impractical.

I have never used the pee hole in underwear in my life. It's a weird design and so much easier to pull my dick up over the elastic than to try to reach in that hole sideways.

While we're on this subject, men should take heed regarding pee etiquette around women:  Never pee standing.  Even if you do manage to get it all in the bowl (And let's face it, a lot of you don't for one reason or another.), the splash effect causes the edge of the bowl to get splattered, and this drives gals nuts, and not the good kind.  (I do realize that there are some pee queens out there who may enjoy this, but most are not.)

So, at your own digs, sure, feel free to pee standing up.  I do.  But when a gal is over, and especially if you are at her digs, be seated.

And of course, be sure the seat (Both of them) are down after you're done.
