The fluid from "squirting" is made in the tissues that would have become a prostate, if the woman had developed into a man.  Stimulating the g-spot (which is this vestigal prostate) makes it produce the fluid, and then a partiularly intense orgasm makes the g-spot contract and squirt out the fluid, just like the prostate does with male ejaculation.

Some women say the first few times they do it they think they're urinating, which isn't suprising since both urine and the squirt are coming out the urethra.

Everything I've read about female ejaculation indicates that the fluid being ejaculated is being released from the urethra, which is the same hole.  The fluid itself is (according to those sources) not from the bladder, though.

This of it this way -- the vagina is relatively big, and relatively flexible.  Even with a fist up there, the skin is flexible enough that any fluid pressure is not enough to make it squirt (and even if it did squirt, what would be the driving force -- the uterus?!?).
