Lets say you are in the mood for some special attention.  Do you look at who has schedules posted? Or do you do a search for what you want?

I have a list of "favorite ladies" that I peruse and call those in the area I am going to be visiting.

If I can plan further ahead which is not as usual I may choose to search the reviews and try a new adventure.

new, or touring talent. I avoid certain traits like the plague.

I run into the same issue where all of a sudden I have a small window in my schedule and only have 1/4 hours to schedule.  So I find myself using APA's because they're convenient.  
When I know I can schedule out, I'll start looking for someone based on what particular itch I want to scratch at the time. They have to have at least 5 reviews from credible hobbyists (10+ thoughtful reviews, not the 'I came, I conquered, I came' variety).

...It's very, very, very long list, so eventually I find a lady who's not having elective surgery or going to her dog's funeral or leaving for Iraq or retiring from providing...

I find I doing 3 things
-am going to my wish list where only fun, "my type"(female!!)legit, reviewed ladies are and try to schedule a week or more ahead. (I have only had to cancel once and that was 3 days ahead of time)
-look ahead at touring/schedules that are on the local NY Discussion board as I am now trying to plan ahead in an effort to control my schedule and spending.  There is a benefit for me in having something to look forward to as well.
-At times I am still a hound responding to my impulses and go for a quick fix local. There are a few choices in NYC that help me with this(LOL)

I don't always do reviews as I have gone for 2nd's.  I'm relatively new to this hobby and have found TER to be an invaluable resource. I don't leave home without it!

I look at my list of "providers I'm interested in", and pick from there.  I update that list fairly regularly, and keep ladies that I clicked with very well on there too.  Based on that, I figure out what I'm in the mood for, and then I see who is available.  It's not a tremendously flexible system -- if I wind up with a few hours free, there are aren't many ladies that I can expect to be available on short notice.  Still, since I don't generally have gaps in my schedule springing up on short notice (let alone be able to afford hobbying at the drop of a hat), I can usually plan it out pretty well.
