If it's insufficient arousal, maybe you need some different stimuli, or some variety.  Also, condoms really cut down on sensitivity, but having a little K-Y inside the condom works sometimes.

If you just can't cum, I don't know what to tell you.

Although not quite the same but similar situation. I am 55 and also takes me about 45min-1hr to cum ( and have not cum in a VAGINA since November and driving me F------ CRAZY). Well I'm glad I'm not the only one who can't cum but at 33?   I get fully aroused / erect but it takes FOREVER for me to finish.  Past lovers / providers think I'm holding out on purpose but I'm not.  Recently,  I stopped mastuerbating for a week and not only did it nearly drive me crazy but it also didn't help.  Maybe you're on the right track and that if I cut down over an extended period that it will work.  

As a side note, I was 13 when I first masterbated, 23 when I had my first wet dream and currently have at myself 10 - 12 times a week like you once did.

My advice is find a lady who really turns you on,and one who you click well with.Stick with her and don't see anyone else.You have to learn to relaxe and forget about cumming.I know this sounds strange,but it is the only way that has worked for me.Just concentrate on pleasing her and enjoy the moment.Eventually you both will find a way to make it happen.It may not be every time either.Though lately, I have to say it is getting easier all the time.Just don't fret over it,that only makes it worse.

I'm a bit over 40 and have had the same situation for most of my adult life.  Even masturbating it can take 15 to 20 minutes for me.  I too explored medical options, finding that there were none.  Some exploration on the net gave me some helpful advice.  The big issue is generally that I have to concentrate very hard to make it over the cliff - with all that is going on in really great sex, that can be difficult.  A couple observations:

1. For me, the less active I am when I am concentrating to achieve orgasm, the better.  As such, Cowgirl is the best position.

2. I have a few "go to" fantasies that really help me get there.  Once I'm ready to go, I start running through them in my mind.

3. I find that for the couple minutes before the big event, it helps to shut my eyes and stop talking.

Not a dis but when the Doctor had  my blood tested for my testosterone level it was very high.And yes I know it is a lot and I have since cut down to maybe two to three times a week and it is driving me F------ CRAZY.And I am coming closer to having an orgasm in a vagina since I have cut back so I definitely believe that my problem is getting better. When I can get to maybe once a week or once every two weeks.Becuase the doctor said that the penis ( believe this or not but think about it before you laugh ) got used to the hand because you know when,where, and how to stroke it and thats what happened.
