When you say 'unwilling', is that because you inquired and were turned down? If not, the lack of performance may not be a function of 'unwilling'. But more a function of 'unaware'.
As for what to do next, if you inquired and was refused, there is not much that you can do. Except, as you suggested, make it clear in your review, that activities stated in her profile or reviews, were not available.
Further, depending on the activity, 'unable' and 'unwilling' may have different or overlapping causes. 'Unable' may just be a function of bad timing (illness, menstruation, injury, etc.). These circumstances may also result in a provider being 'unwilling'.
