As a middle-aged, middle-of-the-road, Midwestern escort I like to think I can speak not only for myself, but for the many ladies I've met over my decade or so of providing.
Well, for starters, they win monthly awards here.
First, it's only a small percentage of providers as a whole who even POST here on TER, let alone the few of that lot who wind up winning the monthly popularity contest. Yes, each Ms. TER gets placement and publicity that cannot be bought, and some free VIP days. But there's a downside too. Perhaps not for all ladies, but at least for some. Secondly, many businesses give monthly/quarterly/yearly awards for sales, productivity, and customer service satisfaction. Hardly unique to providers. 
They have incredible amounts of free time, wondering out loud what it would be like to have a curling iron jammed up their pooper or putting sexy playlists together to fk their John's by.
I hate to give away any secrets, but has it occurred to you that posting on TER is a form of marketing? In other words, posting here can gain new clients. I've had new clients tell me that they only became interested in meeting me after reading my posts. And I'm guessing I'm not the only one.
So, these activities ARE part of our work, for many ladies. Oh, and creating playlists adds to the overall experience, which can lead to more repeat clients. Again, all part of the "job".
Apparently as many as 13 hookers as we speak are getting whisked away by a billionaire in his yacht to Mexico. Wow, that many?! Considering there are hundreds in the Chicago area alone, that's not exactly a significant percentage.
The only time anyone invited me onto his private yacht was for me to fuck his chimpanzee.
I'm an animal lover, but not an animal LOVER. LOL.
Drones are taping them have sex and they have oodles of money to get all types of manicures. Lots of men EXPECT ladies to have manicured nails. So manicures are part of the cost of doing business, IMO. I keep receipts and turn them all over to my accountant.
They never, ever negotitate their rates like every other business in America, and Johns are telling them they love them at an alarming rate. Lots of businesses won't negotiate their rates. My manicurist (since you mentioned such) certainly won't. Nor will my hairstylist. My handyman sometimes will, but not always. Electrician, Heating & Air Conditioning guy, accountant? Nope, nope, and hell no. I learned the hard way about negotiating: give him an inch, and he'll take the whole menu for $50. As to clients professing their love for me? Well, perhaps I need to polish my skills up a bit, because I haven't had one such declaration this past year. :Sniff, sniff!: 
They make hundreds of dollars an hour while they sip the finest champagne, have their feet rubbed and eat the best lobster tail money can buy. I haven't had anyone bring me a bottle of anything that cost more than $50 in a looooooooong time. Hell, looking back over this past year, I can count on one hand the number of gentlemen who brought a bottle at all!
The only guys who offer foot-rubs are the ones who get off on it. Sometimes literally.
Lobster tail?! ROFLMFAO!!!!!!!! (Oh poor, poor me!!) :-P
Of course there is the downside of dragging the bag full of gifts to their cars and wondering whether they should really tell IRS about that $300 tip.
Bwahahahahahahahahahahahahaha! Yeah. right. Gee, I can't count the number of times I wished I'd brought my wheelbarrow with me to handle all the gifts. (If I roll my eyes any more, they'll probably roll right out of my head at this point!)
I hope no one actually believes any significant percentage of ladies receives gifts in that sort of quantity. Or with any frequency.

Some ladies do travel a lot. Why just this past fall, I went to Kansas City, MO AND Green Bay, WI!!! Pretty exotic, right??? Lots of regular folks travel frequently, too. Next time you're on the beach in Bora Bora, ask every woman there for her overnight rate. I've got a hunch that most of the beach-goers will not be providers.
If the reviews here are to believed they orgasm at a rate fourteen times that of an average civie and they have all built web pages that dictate to low self esteem John's what and where to buy them on their "pamper me" page.

While the things you mention can and do happen, the average provider does not encounter such things very often.
Bear in mind that there are far more BP type ladies as well as those like me, than the upscale higher-than-average priced women who may actually encounter such treats on a regular or semi-regular basis.
