I've been using various oral ED medications over the years but have never been 100% happy with them. The side effects partially take the joy out of it and there are also times when the medication at a medium dose does not want to take well.  
Had a visit to a specialist and did a trial shot of Trimix and got a supply. Works well so far but I've only been using it for two outcall appointments while on work trips.
I live far away from any decent action and have to drive at least an hour if not more for quality incall appointments.  
How do people handle the Trimix injections for these situations? Doing it in the car or a public restroom seems strange. At the incall in the bathroom and wait until it takes effect?
Any advice is appreciated.

I arrive at the incall and greet the lady as usual.  If it's a short appointment, I excuse myself and inject in the men's room.  Usually, I'll need 10 minutes or so for it to take effect.  But if there's no physical stimulation, it will take longer, which is fine.  Then in the usual course, it does its job.  

My regulars know, so I can inject when it's convenient.  You may be surprised by the number of providers who are familiar with this procedure; you may be more surprised by the number who really don't care -- in a positive way.  Do what you need to do - it's fine with them.  

Above all, be comfortable -- and happy -- with it and the provider.   Have fun!

There are a lot of discussions regarding trimix . Besides looking at previous threads regarding trimix you might want to ask the question there to get a timely response.

Excuse yourself to the bathroom and inject. For me, it starts working in under 5 minutes so the timing is perfect

I would use the ladies Incall bathroom. If you have a needle disposal container please bring that with you as it’s not polite that you leave it in our open trash. Most of us have seen all different type of ED meds. Just make sure that you stop the blood at the injection site. Press down firm so the blood clots and there shouldn’t be any issues.

You saw a specialist in ED issues and got a prescription? You didn't think to ask the specialist about it -- or ask there before here? It's not like it's some odd or reveling question to ask and a specialist certainly should have the best answers for you.

I’m familiar with these medications and don’t mind if you excuse yourself (and am even happy to assist with the process!).

The only thing I would recommend against doing is, don’t plan to have it with you for use only if you need it. Just use it when you get there.

Get a reusable cold pack like the ones made for insulin or a small ice chest for that one-hour drive or you risk degradation to a lower efficacy if it sits too long at room temp.  I only use it to limit the refractory period on doubles, so I generally keep my on-hand vial frozen between uses.  Otherwise, it expires before I can use it all.  Freezing doubles the life of the trimix.  (The compounding pharmacies tell you not to freeze it, but I don't know anyone who freezes between uses and did not extend the useful life of the medication.  It's in their interest to let it expire unused so you have to buy more.  Just make sure you shake it up vigorously before use because it separates when sitting in one place for a period of time.)

Thank you for googling Trimix for me.  
I did discuss with my doctor and he gave me instructions to use 5-15 minutes before I intent to have sex. I did not want to discuss with my doctor that I meet women in hotel rooms and pay them to have sex with me, hence the question here about actual real life logistics.

This is a great tip. Is the decreased efficacy something you experienced before you started freezing it?

I’m super curious about this because alprostadil alone doesn’t need refrigeration, and papaverine also seems shelf stable, so presumably it’s the phentolamine.

asked anyone WHY it needs refrigeration, but every time I order it (a few times a year), it comes in a very sophisticated Fedex overnight cold pack that they also use for human tissue transport that says on the outside, "refrigerate immediately."   The vials come with an expiration date that is usually about 60-75 days, presumably under refrigeration.  Less refrigeration and more time at room temp will shorten the useful life, while I have found that freezing between uses can at least double, and sometimes triple, the useful life of the drug.  

 
New users getting a higher strength than the basic formula also get a vial of the antidote the first time they order in case they use too much if the prescribing urologist knows what he is doing.  An erection lasting over three hours requires an injection of the antidote or else a trip to the emergency room.  After the first purchase, I don't order the antidote anymore because I know exactly how much to use for one hour, 90 minutes, 2 hours, or a little longer.  Othe users I know say the antidote may be frozen as well and will last up to six months.  

 
In an emergency, if the user does not have the antidote, a double dose of Sudafed will work to start the detumescence process and prevent permanent damage until he can get some medical help.  I recommend that all providers who have customers using trimix or quadmix keep a supply of Sudafed on hand that is NOT expired.

I see only a few providers and am not into searching for any new ones.  As I've gotten older I like the comfort of familiarity and walking into a new situation makes me nervous.  The ladies all know that I use Trimix and 2 of them inject it for me.  They like to do it and the results it produces.  Inject, suck on it for a bit and then take it hard & deep!  They love it and so do I!

As you can see, the degradation of the alprostadil is rather swift.  At this rate, you are at 50% efficacy in just 30 days.  This is why refrigeration is a must, and freezing is even better, because refrigeration doesn't stop the degradation, it merely retards it when comparing it to room temp.  Can it still be used after a 50% degradation.  Technically, yes, but you need a higher dose, and that's where the danger comes in because your usual dosage is not going to give you the result you want, so you will be speculating on how much to use, increasing the possibility of an overdose with an accompanying visit to the emergency room.  Trust me, you don't want to be standing at the counter checking into the emergency room because you have an out-of-control boner.  Using more will also increase the initial discomfort from the injection because the larger volume in a confined area can be painful.  After all, you are sticking a 5/16 or 1/2 inch needle into your dick in the first place, which isn't so bad, but then it takes a while for the serum to dissipate throughout the Corpus Cavernosum of the penis.  The more you use, the more painful it is, so keeping the dosage at a small amount is the key, and that's where the temperature sensitivity comes in.

Okay, but the alprostadil injection alone - Edex - is shelf stable and doesn’t require refrigeration. So I’m curious why it’s different in compounded formulation.

The online pharmacy included a travel pack with reusable icepacks. At home I keep it in the fridge. Do you really recommend freezing over regular refrigeration? I don't mind buying at slightly higher costs for smaller supply, but would love to avoid it giving a bad surprise due to lower efficiency. Only three uses in, but love the fact that I don't have to deal with the side effects of oral medication which were surprisingly strong for me.

a couple times a week, one vial will probably not last beyond the expiration date on that batch, depending on the dosage your doctor prescribed, but I use it mostly for doubles, which is about a once-a-month usage rate for me.  Freezing in between uses can stretch the life of the drug to six months with little loss of efficacy.  If you are keeping it that long, even frozen, once it thaws for use, make sure the liquid is still clear and not cloudy, nor should it have a yellowish tint, in which case you should toss it.  

 
I agree with you about side effects with the oral pills, but the bigger advantage with the injectable is the precision you can create for the desired length of time by regulating the size of the dose.  With oral meds, when it's over, its over, and since the pills affect blood pressure for your entire body, messing with dosages comes with added risks.  The initial dose the doctor gave me for the trimix gave me wood for about 45 minutes, so I experimented for about a year moving up slowly until I mastered the dosages for a 60-, 90-, and 120-minute boner.

Because it's no longer alone, it's in a compounded formula where one or more of the other components are NOT shelf-stable.  I think you would be defying the laws of physics to impute a stability characteristic to a compound where ANY of the individual components are NOT shelf-stable.  Wouldn't you agree?

For the first decade with Trimix I was diligent with refrigeration and the results were pretty consistent. Then I would keep a little in the trunk of my car so it would be available on short notice. It was fine for a few months, it was winter in New England. Just recently I was having trouble with anal and with anyone who was fairly tight for that matter. The pressure at the base because of the tightness cause a loss of firmness. Where I was hard as steel before and not so much now. I chalked it up to age and just thought "this is how it is now". Totally spacing on the drop in strength due to temperature, gradually using a little more each injection until I was up to 30 or 40 units. When it suddenly hit me about refrigeration. When I opened a new unused and frozen Vial I was back to being Iron Man and I felt 20 years younger. So keep it frozen my brothers and stay hard!!
