Its a daily pill. Its a serious pill - over time, exposure to Truvada can reduce your bone density. It takes about a month for the initial non-serious but uncomfortable side effects to subside. Its also not a cheap pill. Out-of-pocket, Truvada runs about 1500/month.
It's also hard on the liver. It's a good solution if you're in a relationship with an HIV-positive partner, but seems a bit extreme for hobbyists given you're still susceptible to other STDs without a condom. If I was a provider, especially one with a solid plan to get out of the industry in just a few years, and I had no other health issues like drug or alcohol abuse, I'd probably be on it. PrEP is also very effective in cases of accidental exposure as a short-term treatment.
Yes, I drink like a fish, and don't want to be "the incredibly shrinking man" as I get older. I can't help getting old, but old and frail is not what I have in mind for my "golden years"
I do agree there are some people for whom the benefits do outweigh the side effects, but it's not for me. Not in its present form that is.
There's been a controversy brewing about this across the pond for months, over whether the NHS should provide this. I'd think it would bankrupt them. There's been a whole series of articles about it for months. Below is the most recent (I think). These particular articles don't seem quite as convinced as you in terms of effectiveness, claiming it reduces risk by 86%. Still, that's huge obviously.
to stick with condoms. (Not to mention so many other STIs) Like fearghas said, it's a good thing and it has it's place, but I don't see widespread utility for hobbyists and providers (except perhaps super high risk types.)
