I am prompted to post this after just reading a review by a guy who said this was his first provider. He asked to use her mouthwash (it was more than an hour drive) but did not ask to use soap for washing his hands. She had to ask him to wash them.
What's the big deal if you just took a shower before driving to her incall? The answer: a study of a few years ago found the "germiest" places were gasoline pump handles, which means that you transfer germs to your steering wheels. It does not matter how well you scrubbed off in the shower, you are arriving with germy hands after driving. Ladies always express appreciation when I tell them that is the reason I want to wash my hands in their sinks. My Slogan: "My Hands Get To Do Lots of Roamin' Because They Have Been Scrubbed"
"You want to put your fingers where, exactly?" I'm usually under an hour from home & generally have the luxury of a shower immediately before I leave... so I generally don't re-shower (& have been complimented on being fresh & clean). But I've even brought my own soft scrub & clean towels... for hand washing. Gas pumps, door knobs, elevator buttons, rest rooms... betwwen home & her location. I don't blame the gal at all.
...use hand sanitizer after pumping gas. I don't even want to know what kind of germs are on those pumps! (lol) I'm not OCD when comes to washing hands, but I do wash them quite a bit.
I have heard people ask if an envelope is necessary for the gift. I think it helps to prevent transmission of germs if you minimize contact with the money

In the 80's a lot of bills were found to have trace amounts of white powder from being rolled up. Even though electronic payment is in vogue and even some ladies accept crypto-currency (bit coin) as payment, nothing beats the greenback in terms of immediate gratification. However it does harbor "some" germs, but they are harmless if hand isn't washed with soap and water for a minimum of 20 seconds. Under ideal conditions a bacteria can divide every 20 minutes. Using ordinary soap is sufficient in eradicate 99% of germs on your hands. (remember underneath your fingernails where germs live too.
