I am not sure what I am doing wrong....  

I have this vision of exactly what I like.  Voluptuous, curvy, MILFy type woman, age 35-49, in sexy dress - meaning spandex or form fitting pants, or tight skirt, stiletto heels, tight shirt, very made up.  Smoking is OK as long as it all white 120's.  I make this request when i make the appointment.

I've seen 1 middle age woman in nice work dresses (she was one of the best times ever but stil wanted the visual), a 20 yo B cup (at best) wearing Capri pants and flip flops, and a 25 yo in sweat pants with OK boobs under a t-shirt.  

I am such a softy I take whatever comes and pay...

So what am I doing wrong? Do I want too much?  Should I drop my vision requirements?

TER is highly populated with just the kind of ladies you're looking for.  Before anyone can tell you what you're doing wrong, you're going to tell us what you're doing.

I can't see asking for a voluptuous MILF and getting a 20-year-old spinner.  You're not clear with your expectations is all I can think of.

Is this an outcall to a hotel by any chance... if so, most ladies won't walk through a hotel lobby dressed like that, as it will draw unwanted attention. If you're asking for a wardrobe request and she says OK, she should at least be bringing the outfit to change into. Although I agree w/the above poster..if you're interested in MILFs over 35, how did you end up w/2 ladies under 25?

The best I can figure is the agencies doesn't care what the request is - they just send whoever is available.

And you can't get "MILF" from a girl in her twenties.

Look for a woman in the age range you are wanting with pictures that match what you are looking for and then let her know you'd like her to dress like her pictures.

Work dress? Flip-flops? Sweat pants? Please, thigh highs and sexy high heels are standard fare for me without any special requests.

I have to have guys ask me NOT to wear them when they like bare legs or bare feet. Feeling sexy is fun and I love it.

Do your research here instead of taking potluck with an agency.  From your post, it's pretty obvious you are not doing that.  That's what you are doing wrong.  I'm sure there are plenty of girls out there that fit your criteria, but you have to look for yourself.  

You do not want too much.  There is no need to drop your visual requirements.  Find a lady near you that fits your desires, research her (read her reviews), maybe contact a couple of her reviewers for additional info.   Then, if you decide you want to see her and make the date, tell her how you would like her to dress for your date.  Most providers will accommodate reasonable wardrobe requests.

Just don't settle for something you don't really want, there is no need for that.
