From what I understand, it simply means that if a provider offers multiple services at different rates, like say FBSM, BDSM, GFE, PSE, dinner, lunch, etc she will custom make a specific, unique package rate for all of your needs, but different women may have different definitions of those words of course. Always best to ask that specific provider how she defines that term, however, as I am sure there is some variance.

If it's not an agency or a pimped provider,  most will customize  to their level of comfort and size of your wallet.

Honestly, custom or bespoke dates cater to clients who are not concerned about budget. I plan all of my dates with an eye for each client's specific desires, but bespoke arrangements tend to involve more activities out in the world, taking trips together, upgraded suites or resorts, and more communication between dates. Instead of paying an hourly rate you will receive a quote for the experience as part of the booking process.

Usually a custom date depends on what the female has to offer.  

I had one recently.  I asked in the LA Boards for a pampering session.  What I really meant was, Here's a list of things I might be interested in, when I get there I'll decide.  So be ready for it.  Some days I want some PSE, or greek, or an amazing bj.  Other days I just want a long relaxing massage.  Or to put my head in someone's lap and take a nap.

And if a SP doesn't mention custom or bespoke dates, is there a general convention as to whether it's reasonable to ask if she would be interested?  Although one could think my question could be somewhat odd, consider that negotatiating rates is generally seen as gauche and it's in this context that I ask the question.

Exactly that.  I gave her a list.  I made sure to get her consent on everything.  She gave me a price which worked for me.  We swapped some message back and forth to make sure we were both understanding.  Only headache was parking at her location.  But it's LA.

I think it really just means crafted to what you like with a rate that is possibly up for discussion depending on what you are doing.

I asked for a different sort of scenario to a provider I am very friendly that she didnt have the fee listed for it and she said she would work up a "bespoke" date/rate for me. This was 5-6 years ago and that was the first time I remember seeing that word being used in this lifestyle.
