Would a provider prefer 15 small bills or 3 large ones? Also, what might be a nice small gift?

Large ones.  Less of a tendency for client to do an "ooops".

Assuming you dont know or havent met her, most providers will appreciate almost any token of thoughtfulness from a client.  A cd (romantic is nice, like Andre Bocelli), roses, chocolates, pretty stationary, a victoria secrets gift certificate.  Y'know, "Girl Stuff"

bills are best, as they are more difficult to spend (IMO).  As for gifts, it depends... I've gotten some great ones, from roses to gift certificates for books, to a gift certificate for a massage (man, that was FANTASTIC), to some hand-tied flies (caddis flies!) for fly-fishing.  One guy even gave me a ticket for a steak dinner at the local Civitan (or Ruritan or something) charity fund-raiser.  I'm a vegetarian, but wasn't that sweet?  I've also received a fair number of sex toys as gifts, but frankly, that's not my favorite because I feel obliged to categorize them as "from Joe" or "from Fred" (permanent marker, perhaps?)and bring them along next time.  

I guess my advice would be to find out her interests and get her something along those lines... for example, I wouldn't recommend giving your ATF LA beauty queen escort hand-tied flies... my guess is that gift might go unused and unappreciated.  BUT, an LA beauty queen might just like a gift certificate to a fancy boutique.

Happens-  but usually folks love those sorts of gifts.   I will bring partiaBLES if I know the provider is into it...

And we can practice our casting techniques together.  Here in the mountains, that probably means something entirely different than what it means in Southern California...

I tell you, Nicole, if you came out here, you'd have to cut your rates in half, BUT, judging from your pictures, you'd be wealthy in no time at all.  I don't believe there's ANYBODY who looks like you "out thisaway."
