Could a space for a providers TER id be added to the review form?  I would think this would make it easier to add the review to the database.  If you see a provider who has already been reviewed in TER and has an ID, you could just put in her ID and be sure it was linked to the proper providers history of reviews.

That would be especially good if the ID input would override the need for all of the individual info if they are already in the database.
