Several possible reasons starting with you. If you have bad hygiene in any way or poor dental care providers are going to pull back. If you have a weight problem, she may have been put off by that, letting her know beforehand would help. She could be a YMMV provider that should consider quitting the business. She could have an SO that does not want her seeing some types of clients. There are many other possibilities, my advice is to forget her and move on.
