Have you tried Tiffany in North County? Don't just take my word for it. She has a good reputation as a fit and gorgeous provider. She's not keen on having reviews posted, but there have been more than a few discussions about her on this and other boards.

Interesting how people think that UTC area of La Jolla is in NORTH county. I live in the middle of North County and would have to drive 25 miles to get to La Jolla.

Well there's Tawny in Encinitas soon to be farther north. She may not be everyone's cup 'o tea because she's petite but I plan to visit her again.  (My review of her seems to be stuck in the ether.)
