I sought guidance, posed this same question and got some decent advice from both providers and more seasoned hobbyists a few months ago.  Of course everyone should go with their own personal feelings, but here is what I think works:

After having taken care of biz (during that getting to know you period) I present a nice "Thanks for meeting with me" card with one or two sensible gift cards in it.  I find it a nice way to let a provider know that you appreciate their time and effort, it also sets the stage for a positive attitude and mutual fun.  Gift cards are also easier to transport and handle.  It feels awkward in the lobby or halls carrying a bottle of wine or gift box around. (Don't like to feeling like I'm wearing a "Here's the John" sign on my back)

Before departure, if the service has been up to par, I think its appropriate to reward excellent excellence with a tip.  For convenience, I already have it in an envelope (with a smiley face on it) in the pocket of my sports coat.  I simply place it on the table with a sincere thank you.  

Like going out for dinner (my other biological need , I figure these extras into my overall entertainment budget.  It make for a better experience for everyone.  I'll PM you with what amounts work for me.

I agree with you on that one issue about going overtime.  And especially if at her invitation, provided there is no one right behind you in her book.  That clearly deserves a tip.  Also if the lady really shook me, leaving me dazed, confused, and weak in the knees, she has gone past the minimum and deserves a tip.  After all, a tip helps ensure you are high on her list the next time she comes into town.  Lastly, tips are sometimes discussed among the ladies on the providers-only board.

Trace, "Actually I think the only time I tip is if there is absolutely no clockwatching and have gone over."  Not a clockwatcher means that she doesn't short change your time and give you the full hour you paid for.  Don't take advantage of and expect her to go over time.
