Or do I need to request to have to attached?  Also, how come some of the reviews have stars and others do not?  Is it because they are new?

From the Self-Help Center:

Click on "Contact Us" on the bottom of the page and request to have your reviews linked to your user name.  Make sure to include your provider ID number or a direct link to your profile in the message.

Use the contact us button and ask to have your review profile linked to your username. Be sure to give them your profile ID number, or a direct link to your reviews. Once you have the link, every time you post there will be a link under your username saying "see my reviews." Having this link also gives you the Free Limited Provider VIP, which allows you to read the juicy details of your own reviews and gives you access to the PM system.
