How does a provider/entertainer access the whitelist and write about me?

I'm going to answer as if you are legit, but your review of Misty makes me wonder.  Giving the benefit of the doubt:

First of all, you should consider yourself really lucky with Misty.  Her phone number comes up as a bait and switch agency where you are more than likely to get ripped off than have the experience you had.  Also, the descriptions of the lady between you and BeefEater suggest that the reviews are not the same girl.  So, you should forget about her giving you a whitelist referral.  Her link is broken anyway, so she probably has moved on and doesn't even know she has reviews on this site.

Susan has been around since 2002, so she should know how to give a whitelist referral, although I took a quick look and she doesn't seem to have given one to any reviewers on her first two pages.

Providers do not "write" about you on whitelist.  They just give you a referral or not.  If one gives you a referral, then she is saying that you are safe (ie. not LE, non-violent, won't short the money, etc.)  Click the link for more information, but it will be up to the provider, not you.  You can ask, but she has to actually give you the referral.

The FAQ doesn't cover all the specifics.

For a gal to whitelist a client, she must:

1) Be a TER member.
2) Have been reviewed.
3) Associate her profile with with her TER user name.  This is done by contacting [email protected].

Then she will be able to whitelist you.  However you should know, a whitelist entry only indicates that she has seen you, and is indicating that she would do so again.  There is no place for her to write about you.

The whitelist is pretty much a yes/no kind of thing.  You meet a provider, and during screening, decide to  give out your TER handle.  Then, when you see her, and she decides that you're an acceptable hobbyist by her standards, she'll give you a whilelist reference.  It's not a review of you, just a statement that you're safe to see.
