Every provider is different. While I generally allow some shower time (within reason), this question was posed on the Chicago Board and the consensus was that it was counted as appointment time.
Providers shower before your arrival. They have fixed their hair, applied makeup, and have primped and put on something sexy so that they are ready when you arrive. As such, they are not likely to join you in the shower.
If you take a quick shower when you arrive, especially if you have not showered since the a.m., the appointment will go much better. The key is to soap up well, especially your privates and backside and rinse off. The same is true if you shower after you have finished. I had a client spend almost 30 minutes in the shower and had to knock on the door and remind him to hurry up. He made me late for a dentist's appointment as I still needed to shower and get ready.
Just be considerate. If you showered immediately before the appointment, tell the provider and she will probably let you slide on another shower. However, if she goes down to give you a BJ and there is an unpleasant odor, she may pass on the oral. Do you really want to take that chance??

I have the advantage of leaving from home. I take a quick, "hit the important spots shower" if my travel time exceeds an hour (or I've been heated for some reason, like a hot car in the sun).
Generally shower time is part of your on the clock time... rarely a lady may opt to exclude shower time. Her business, her choice.
Generally a lady will not join you in the shower unless it's part of the play... or an extended date...

If a guys is already showered then I don't ask him to do it again that'd be pointless. If he does want to take one beforehand I dunno if it's bad manners or not but I let him know as long as it's a short one I don't count it into our fun time. It depends.
