Yeah I know its a stupid question. But if you indulge in a threesome, it is ok to put the donation for both ladies in the same envelope, or do they prefer to receive one each?

I always use two !

Give it to the lady who you booked the appointment through.  She will take care of her partner.

I've never seen a listing for doubles that specified $$$/hr for Sally and $$$$/hr for Jane.  It's always been $X/hr doubles with my amazing partner.

It is OK, however to tip each independently after the date.  I'd keep the amount the same for both.
