Yeah I know its a stupid question. But if you indulge in a threesome, it is ok to put the donation for both ladies in the same envelope, or do they prefer to receive one each?

one I actually split the notes so they both got the same denominations and divided it accordingly

Give it to the lady who you booked the appointment through.  She will take care of her partner.

I've never seen a listing for doubles that specified $$$/hr for Sally and $$$$/hr for Jane.  It's always been $X/hr doubles with my amazing partner.

independently, which is pretty rare, but not unknown to happen.

It is OK, however to tip each independently after the date.  I'd keep the amount the same for both.
