A lot of guys who make appointments this far out like to "keep in touch" up through the day of the appointment, creating a LOT of work for the poor girl who ends up putting several unpaid hours worth of work on the hope the guy will actually show up some two months later.

I would most definitely cut her some slack about her response time about an appointment this far out.

I provided all the information requested two days ago for an appointment including name, phone, hometown, and refs. The appt is a couple of months in the future because I'll be traveling.  
This particular provider is well reviewed and has two email addresses on her profile in addition to the form on her site.

How long should I wait for a reply before assuming she's not interested? Or, maybe she's not that diligent checking inquiries from her site. Should I use one of the email addresses (or both) and reach out that way? I'm not comfortable with crickets after 72 hours. Any advice would be appreciated.

... you might want to allow more time. The provider needs time to complete her screening, and that can take time as it involves checking your references.  And this is peak summer vacation time, which can delay things like reference checks.  

What you might do is, if you haven't heard anything in a day or two (and consider this is the weekend now), send an email to ONE of her email addresses (review her website for contact preferences) and explain you're following up on your request from X date for appointment on Y date. If you have a deadline for her response let her know what that is--and that if you haven't heard from her by then you'll assume the appointment is off.  Be very polite about all that... emails like that can easily be taken the wrong way.  And if I were you I'd make the deadline generous, since you have plenty of time to line up an alternate provider if needed.

#3 one week ahead and #4 forty-eight hours in advance.  All emails should be brief with just enough dialogue to let the provider know he is planning on following through on the date/time.

Providers don't want email buddies.

There is no rush here. I would give her a week or maybe even 10 days before following up.

When confirming the appointment, the provider can say something like, I'll send an email (pick a period) before the date to reconfirm; no need to send me any more emails before then, unless your plans have changed.  

But there's providers I'm aware of who like corresponding with clients before dates.  

Frankly, we have no idea why this provider hasn't responded yet.  The mystery for me is, why does the OP seem to need a response so fast for an appointment two months away?

I welcome advance booking however a guy in particular has booked a date three months in advance.
I made the mistake of giving him my unpublished phone# he has proceed to text me every few days and on the weekends.Besides that he has changed the appointment date due to "family emergencies" three different times.
Now he is not sure if he is coming to town at all.I agree with you about some guys who book in advance hopefully the op is not one of these guys.
op should wait another day or so and send an email to one of the email addresses and see what happens.

There is one other piece of advice I would give you.  Pick out another 2-3 providers as back-up options in that area if your lead option doesn't pan out.  I've had mixed success with booking anything over 1-2 weeks in advance.  In one instance, I booked about a month in advance because it was an out-of-town girl I was interested in and I needed to accommodate both her and my travel schedules.  She had to cancel on me the morning before the appointment due to some unexpected family stuff.  So I had scheduled a month in advance only to see it fall apart a few hours before.  I ended up finding another provider to fill in and it went fine, but it was a mad scramble to find another provider in time.  If I had a couple of back-up options already picked out, it would have been less of a cluster fuck.  Good luck!
