You call prior to the appointment (either setting it up, or to confirm), then call again when you reach a certain point.  In my experience, the second call is usually done when I've reached the incall location, either to get a hotel room number, or an apartment number.  It also lets you know the lady is ready for you.

at a certain time.  I waited there and got a second phone call guiding me a few blocks away and into a parking spot and down a long walkway to an elevator and then down a very long external hallway to her room.  We were on the phone for 10 minutes with long, weird silences (her English was limited).

Another two call system: I was given an intersection (in a phone call) and told to call her.  When I did She said to pull across the street and park in the spot with the geranium by it.  I did. Another brand of two call system:  I called and left a message to make an appointment.  I got a call back twenty minutes later from a guy asking for "Bob".  My name isn't Bob.  30 seconds later I get a phone call from the lady about the appointment.  I gathered that the "wrong number" was some strange form of checking my cell number.  Everyone was ID blocked except me.

Yet another kind of two call system occured when there was only one phone call for all the details and address, I showed up and got naked and when she put her mouth on my cock I called out very very loudly!  Then I put my tongue on her clit and she called back.  It was my favorite two call system.
