Most commonly:

1.  That the provider is no longer working in this business (retired), or has decided to take an extended vacation or sabbatical from working, and has requested removal, temporarily or permanently.

2.  Provider has requested removal of her data from TER, perhaps to keep a lower profile, for example.  She may, for example, not want to attract new clients but will continue to service her existing ones.  I don't know what the policy is if a provider asks for this because she has rip-off warnings and/or reviews.  But my best guess is that TER will not grant a request under such circumstances.

3.  The provider has committed some offense or broke some TER rule.  An example would be to ask a friend, not a client, to write a bogus review, or if she bad-mouths TER on some other Board.  In this instance some providers are eventually reinstated, but others perhaps are permanently banished.  TER is pretty good about policing their site.  

BTW, members are subject to banishment also for breaking any specific rules, like posting bogus reviews.  You should read TER's regulations at some point, if you haven't already done so.
