A lot of ladies require real name...  which they might google or check your office to see you really work there.  
I have seen rare instances where it is expected to show your ID briefly on arrival.  
If you can not tollerate this you have every right to refuse...  as she has every right to decline the appointment.  It's what ever she needs to feel safe to be alone in a locked room with you.
Many ladies do not require this...  it's up to you.

Have you guys ever met  a provider requiring checking your ID to make sure your name matches? so basically I have to give out my real name. Is this a warning sign? would like to hear your opinion on this. Thx.

Exactly right.  I've had one date where that info was required; I did it and it was fine.  Is it possible it could blow up in your face?  Sure.  But all of the verification requirements in this business are risky.  Risk is the nature of the hobby.  If you can't do it, don't.  But accept the restrictions that implies..

I want to be sure that that the "Bob Smith" I screened is the same "Bob Smith" standing in front of me.  It is conceivable (and evil, in my book) that someone would use his buddy/coworker/enemy's info to have me screen and be someone else entirely.  If, god forbid, I should wind up in little pieces stuffed into a suitcase at the bottom of some river - the cops need to know where to start looking and who to ask about having been the last person to be with me.  Once I return safely from an appointment, all personally identifiable info is destroyed as neither the gent nor I need the potential exposure that keeping screening info affords.  But until I return safely... its my job to protect myself, even if it should be after the fact and from the bottom of a river. ;-)

Happens quite often actually.  If you are bound and determined to remain anonymous it is going to severely limit the number of quality ladies that you will be able to see.  

I've been giving indies and agencies my real name for ten or fifteen years now and It's never been a problem.

With RS2K and P411, the ladies are suppose to check your ID to make sure it matches.

I must admit that ladies that check ID are the exception, rather than the rule. For one, if the guy was LE, by the time she opens the door, it is too late. Ditto's if I was a creep. Only ladies that take security to the next level (have to check-in with her body guard to say she was paid and you are ok) could defeat a creep at that stage.

picture matches, then 99% certainty that I am not local LE (nothing is 100%).  BTW, I covered up my address with my thumb; all she saw was the name and picture.

Yep, ladies who use RS2K, are supposed to check ID to make sure the person that gave them the RS2K info, is the same gentleman that is in their presence.

But once you call for an appointment don't they know who you are and where you live?  So why object to showing your ID?  It's too late right?

I require clients with no reviews or provider references to show be their driver's license on arrival. I don't care if they want me to call them Mike and their real name is Sam. The purpose I feel is it you are going to show me your real name, address, and driver's license number you're not gonna rip me off or hurt me.
I don't take down any of this information and won't remember it if we have a good session. If a man refuses to allow me to see his ID I won't see him. He has every right to say no and want to be discreet, but I have every right to be safe as well! If a guy is uneasy about showing me his ID I offer to show him mine as well to prove I mean no harm. I like to be newbie friendly but I also need some level of safety and comfort for my sanity.
I adopted this custom the first time I got ripped off. I ran the guys name through a few checks and nothing came up. It turns out he opens 10+ emails accounts with fake names and variations, sets up meeting with provider then rips them off. He is constantly making new names and constantly getting a new prepaid phone. He literally makes a hobby just out of ripping girls off. If I would have asked for his ID I'm sure he would have refused and I would have not been ripped off. This is my personal reason.

I don't think this request is odd, but remember you have a right to refuse! But I would generally trust a provider who has positive reviews and asks this of you.
I like it when clients use their real first name. Just as I use my real first name. Last names don't matter and I don't ask.
