I made an effort to write a review but since I didn't know the provider's email I was unable to submit it. How do you fix that?

However, a means of contact is required. Either an email address, phone number, or pager. Of course pager is kind of silly now as noone uses ghem anymore (but they were still common when TER started). In any case you do need at least one of those fields filled in, but not all.
