So I wanted to request an ok from a provider on P411.  While I am filling out the form it states that my phone number is required.  Now when I spoke with the provider I used a burner number.  Is it fine to put that number there or do I need to put my actual number?  What is the purpose of them asking for this too?  Thanks.

That way it'll match up with her records.  

The purpose of asking her for the Okay is that there's no other way to get it. We used to be able to initiate that process ourselves, but the guys have to do it now.
