How do I file a problem report.  I have found a provider with a second list.  Same (some of them pics), different phone numbers, one with email address, one without listing different cities.  It took me aback as the new listing I found had only one (not the one I submitted several months ago - which was a 5 on the service level).  Not sure if she is trying to run from that or what but just think that somehow they should be linked.

Is this something for a problem report and if so, how do I submit one?

From the Profile page (probably the new one with only one review), click Report a Problem from the tabs / buttons along the top (under her name, above "General Information"). I always include the conflicting TER ID #s so it's easier for them to find and cross check.  Typically, if TER is satisfied there's a mistake, they combine the reviews into a single profile, usually the older profile, but that might depend on which appears to be more reliable.

Then click on "My Problem Reports" or, if you've reviewed her, to to "My Reviews" and click on Problme Reports.

TER may not approve the problem report.  If the phone numbers in the profiles are different, TER may determined that they can't know for sure that it is the same girl.  It happened to me when I submitted a problem report in the same type of situation.

The provider has nothing to do with how many profiles she has on here. If theres more then one its because someone submitted a review with contact info that was different enough that the staff thought it was someone different.

Once more, it has nothing to do with her, she didnt do it and has no control over it.
