So I've been around the hobby block a few times, but I'm trying to set up a date with a higher end provider. The verification options are 1: full name, employment and employers phone number??? Options 2: is a prepaid deposit to a paypal account.  

Does she has some kind of reviews here or somewhere else where you can at least see if she is legit?
There are ladies who will ask for all of that info and some who will ask for a deposit.
However you do what is comfortable for you if you want to see her bad enough provide the info.
If not see a provider who doesn't require all of that info or a deposit.
There are guys who will do this and some who will not everyone does what works best for them.

That's what I would do, since I don't do deposits or give my real name. It's a free market, the provider can ask for whatever makes her feel comfortable and then the hobbyists can respond accordingly.  

If a deposit is enough to verify without personal info, she doesn't need or get personal info and I would never give a deposit to someone that may or may not be there when I arrive. It's up to her whether or not to accept the info I'm willing to provide, and therefore if a meeting is going to take place or not. The other option like HH said, move on.

 just humor her. i have first n last on my form, I have alot of guys i caught them , and discover real name, one guy used a certain name, and with all the providers, we all reference check him by the name john do for example. i was in his car as always i push boundaries and like to be closer and friendlier than is advised and have the clients drive to do errands and other rides places, which i guess isnt in the hooker hand book of laws. i think in section one paragraph three, it reads though shalt not go in a clients car with him, before or after a session.

 create a fake name, or just submit form to your comfortability, with a note in fiill in blanks, where it say work info, say  i am not comfortable with that,  
 but really want to meet and am willing to instead show my license.

 i know for me i have a cover story i go over with client in question, i call and say hi mr so so has applied for a loan with us at strikan loans company we are just calling to verify that mr nogin is in fact employed here at such n such.
                      keep in mind when we are heavy handed with screening its for me at least say i have a guy wife threatening to murder me and she also gave my name to popo. which happens to us all time youd be surprissed on our end the crazy reasons we may have goin on for makin us require extensive screening!
 keep in mind all the possibilities on her end she can be coping with in this biz that make it so she must be vigilant on protecting her assests, family and home, .  
 there is a saying i love "TRUST NO ONE' another is
 " A FOOLISH ASSICIATE IS MORE DANGEROUS THAN A CLEVER ASSOCIATE"
 if your foolish enough to believe for one minute a well reviewed successful high end lady is gonna take your crappy little pocket money deposit and run.  
 or worse give a second thought about your work info after her initial screening to make her safe.  
 you gotta definetly ditch the ego. she dont care bout your info. she dont need you 200 deposit or 600 or 1000
 trust n believe if she were a rookie or scam artist and this was her first godam rodea. like it apparently is for you . lol.  
 she wouldnt be capable of goin on in such a manner if she wasnt conducting herself in a certain matter and it was going smoothly, she d very soon go out of business!
 just do the hoops she asks of ya and get ready to hold on to your seat.  
 when you have alot to lose you tend to wanna protect your self  . when you got nothin to lose you post for shit money on bp and host in cockroach motels , and see dirty careless rookies. xo

And deposits are used as an indication of how serious you are about the date specially if she is blocking you time for a longer engagement.. Should you go ahead and do it? it all depends on how bad you want to see her, but keep in mind that in all honesty, there are providers out there who ask for that info for safety purposes not because we want to be intrusive on your personal life.. We will never use that info for any other purposes aside from screening... It all depends on the character and reputation of the lady.
If you don't feel fully comfortable then move on and find someone who is willing to accommodate what you wish to reveal or do..

First, never give out information that you are not comfortable giving out.  Once it is out, there is no getting it back.  Think carefully, then decide.

There have been a few of those "high end" providers who have kept deposits when they cancel, some with multiple hobbyists. No, it is not wise.

Other than me not giving her a deposit, my full name, my employer and my employers phone number, she sounds like a perfect match for me.

Special Circumstances...  extended date where asking to block off & turn down others for multiple hours or days like overnight.  Travel expenses like airline tickets for her to come to you.  I've never encountered such a circumstance & probably never will.  

...you know about P411, Date Check, and so on. If you don't have one of those getting one would help. Does she take any of those? Is she a member of any?

have not used it much to know what they are talking about.  

To the OP, while many women will advise you that a deposit is like leaving a credit card with a Marriott to ensure you show up. I, and most other guys here will disagree vehemently. I would NEVER send a deposit "in lieu of screening" it doesn't make the provider a bit safer and it opens you up to the possibility of getting screwed, but not in the good way.

Some very good advice has been given to you by some very experienced mongers, I would suggest you follow their advice instead.
