First of all, let me check if your 100% certain you can tell its an std (don't read if you're eating): e.g., herpes lesions (vesicles/pustules), gonorrhea thick white and smelly leakage (Yuck again), syphilitis penis (the single lesion or the Me. Marcus syphilitis one), etc. Anything else is not so clear a call (smelly smegma, irritation from teeth, acne).
For argument's sake, let's say you can. Then, I would advise you to cancel the appointment as soon as you see the evidence, expressing a note of concern. At that point, there should be an almost complete refund unless you think there's a show up fee (say, if for some reason it took 30 min before looking at the penis). That way, not only do you save yourself from exposure (eg, herpes is contagious even with closed lesions), but you also negate any good justification for a review. Let him dare to complain then, since he got his money back, you didn't provide any service to review, and you helped point out a serious medical issue. Btw, any guy showing up with herpes, syphillis, or gonorrhea has got to sort of know something bad is going on, so if he tries to pressure you to continue, he's a creep.
