I have an appmt set up and the provider said she'd email location out 3 days prior to visit.  No email was sent by her and now am not sure if still on.  Would you contact provider again or wait for her to recontact you?  Could have a problem on her side?  Her instructions were clear that she'd recontact me with the location, so do I contact her or play it safe assuming she needs to reschedule?

Many providers will not provide their exact location of their Incall 3 days prior to the appointment sounds a little unsafe to me.
If you are in doubt about whether the appointment is still on send her an email asking her to confirm.
Usually a provider might give you a general idea of where she is located and then provide her location the day of appointment time.

I would not want to give my hotel out 3 days before, because anything can happen in that 3 days including leaving the room for whatever reason. I would contact her and see what's going on, but I hope this is a reviewed provider because her tactics seem odd to me.

every provider should let you know the location the night prior to your appointment..anything before or after that tend to make me doubt it will come to pass..

Not uncommon and they've always worked out fine

...you're saying that she siad she would send you the location 3 days prior to the date and that it is now within 3 days of the date and she has not done so.

What the others have said is true, it is unusual for a provider to give that information out that early unless you have seen her a few times and have built some trust. However it isn't unheard of and everyone operates their own way. Since you have posted this on the newbie board I'm figuring this will be the first time for you to see this provider. If it is within 3 days and you have still not heard from her by the time you read this go ahead and send an e-mail simply to confirm the appointment and inquire about the location information.  Say something (without questioning or blaming) like, "looking forward to seeing you and awaiting information on the location".

I will add here that it is important that you have checked to see that this provider is well reviewed and many of her reviewers have written many reviews of providers. If she meets this criteria then you should be fine. Newbies should not see providers that can not be verified by reviews and her ad and/or website are verifiable as legit.

I don't need to know where she is, only where she wants me to call from.  I've had intersections, gas stations, coffee shops (I like the best as I can relax while I wait).  Call location should be within a couple of minutes drive to her.
