I don't think guys will pay extra, esp if it's an outcall.  If you enjoy it, just let them make you do it.  Personally, I would not pay extra for a gal to squirt, but that's just me, some guys might.  But, if they know it's easy to make you squirt, I doubt many will pay extra.

nothing I would pay extra for.  I have been with several squirters and the experience is pleasurable but it is not something I seek out.  DFK and BBBJ, on the other hand, are musts.

I would pay extra, at least once. I've only ever seen video's/web cam. I think that it would be fun to be a part of.

I can from either.  I get extremely turned on and can do at will. I will ask my partner before hand, what their take on squirting is, so I can hold back if necessary.

I'd get a facefull, a wet hand, or a wet crotch. Whatever it was, the bed got soaked. I loved it.
