Any of a number of reasons are possible, but maybe she wanted one more reinforcement to bring you back for more.  A lot of people will tell you that the best tip is another appointment.

You had a good enough time to want to pay her more than she requested.  Add a little more to it, and schedule with her again.  If she says no, then it's something else.

I am fairly new to the hobby and have found a lot of good advise here on TER but evidentally I still have some things to learn.

Recently I was seeing a lady for the first time left her donation in an envelope which she collected at the end of the visit.

Now, I had an awesome time with her.  So I made up my mind that I wanted to tip her.  Now I have read suggestions here on TER on how to go about tipping.

So as I was walking her to the door I reached in my pocket and palmed my tip.  After a goodbye hug I presented my hand with the tip.  

Upon seeing it she said that her fee was all inclusive.(So did her web site and she had good TER reviews so I already knew that.)  So I said that it was a gift that I wanted her to have.  She said thank you and gave me kiss and another hug and left without taking the tip.  What did I do wrong?

Even though the appointment was complete, she could have been nervous about the exchange.  Handing her the money (rather than placing it somewhere conspicuous) probably rattled her a bit.
Or... she just wanted you to keep it.

I don't think you did anything wrong. It sounds like she is just a classy lady.

Yes, I've refused a tip a few times. Once was because he too was a newbie and it just seemed like the right thing to do, another time was because in MY opinion at least, the time we spent together was not my 100% best. He was happy as could be, but I knew it could have been better, and didn't feel right allowing him to tip me.

So write a review for her, sing her praises and tell us what she did that was spectacular - and welcome to the hobby :o )

The lady simply had class.  This businesss is about service.  Good service means not taking advantage of the hobbyists and the hobbyists being respectful of the lady.  Good service means more business.  If you had a good time, see her again and take her a real gift.  It doesn't have to be fancy, it's the thought that counts.  If she has a website, she may have a section other than her rates that shows other things she likes. I just sent a gift (different in each case) across the country to 2 of my favorite ladies just because I haven't seen them for a while and miss both of them.

What I would do with my ATF was stuff it in her back pocket (she usually wore jeans).  I would never hand it to her.  Plus it allowed me to play with her ass some more which was always fun.  ;-)

I already have accomplished some of the advise given as I had already posted a review.

Entirely possible she is a class act as she was in every other way.  Although not sure about marketing as through screening and we talked about where I was from as we got aquainted during the visit so she knew that I was not local by any means.

I will be a pleasure to take the advise and try to reschedule when I get back in her area.  I had already thought about that as a way to tell if I made her uncomfortable.
