If a provider already has several reviews in one city, relocates to another city, changes phone numbers but keeps the same name, would she be considered a new provider?

If she keeps same handle, she just needs to change her information on her profile by submitting a problem report on herself.

Some providers have a legitimate reason for completely changing their identity, e.g. stalker, but others do it to get away from their bad reviews.  I wouldn't let the TER community know if her safety is involved.
