Her website may have instructions regarding the donation. But otherwise, I would recommend to just go ahead and place it on the closest table or dresser without comment.

Re: first time in call appointment

As others pointed out, check her site for any special instructions. If none are specified, just place the (plain, unsealed) envelope with the exact donation amount on the table or dresser, discreetly, right when you arrive (after greeting her, of course). It is also common courtesy to excuse yourself to wash up- she may suggest it, saying "Do you need to use the restroom?" or "I have fresh soap and towels in there" but if not you can bring it up by asking where the restroom is located or just excuse yourself for a moment. This is both a hygeine thing- many incall ladies prefer that the gent showers upon arriving, or if he says he has just showered, at least do the hand wash/mouthwash thing. In addition, this gives her a chance to discreetly check the donation for new clients, without having to count it out or handle cash in front of you, which most of us would consider quite tacky.
