You do not want to ask her anything that is already mentioned on her site, or she'll think you didn't bother to read the site carefully, and that is a big turn-off for providers.

Then tell her exactly when you would like to meet, for how long, and whether in-call or outcall (if there is an option).

Do not discuss sexual activities, and do not discuss payment.

Also, tell her the best way to get in touch with you.

Tell her where you saw her ad, or how you found out about her.

You can give her some personal information about you such as age, weight, height, etc. if you wish.

The email should be fairly brief.

How should I email a provider since i would like to see her ? what should I sound like hi my name is such an would like to have an hour of fun with you.. Available this weekend coming up???  Overall what questions should I ask her as well in the email

don't say an hour of fun.  Just say an 'hour of your time'.  She knows why you are contacting her.  Be a little more specific about the day and time, rather than just 'this weekend'.
