how do you ask for special treatment (you know, h.j. and etc without actually asking it directly.)

The girl will bring it up.  In my experience, and what I've heard from others, the girl finishes the massage then asks if you want anything else, or something like that.  Then you hand her five more twenties.  Maybe more in NYC.
