I am a total nube at this so apologies in advance but the Asian provider reviews show rates that vary from $200 to $400 per hour.  

Is the rate listed on TER the rate just to get the table shower and into the massage room and then you negotiate the full service fee?

From what I can figure out the $350 to $400 Asian places are full service for the price asked based on the reviews. It seems the $200 places are the places where you need to add about $100-$150 for the full service?

Also, is it always one hour only? Can you request 90min or 2 hours at the Asian places or is that bad manners and not done?

I don't mean to be rude or unkind but I'm not sharing this info in an open forum with someone without reviews.  Others may feel differently, that's up to them.  

Best advice I can give you is to read the reviews, make some visits to places with various prices and note what your experiences are like.  I think you'll soon find answers to all these questions.

And after you do that, please post reviews -- that way people will feel more comfortable answering your questions, but as I said I think most of those you've asked here will be answered if you follow my suggestions.
