I did a search for Minnesota providers with many of your parameters (some of them aren't easily searchable, not in a single pass anyway).  I didn't include the not-that-important criteria.  Didn't include eye color of course as that's not searchable.  Didn't include ethnicity or body type (can't do what you want in one pass).  Don't know about drugs/drinking.  COB isn't searchable.  

Got zero hits.  Even if I take out cup size, still zero.  

That means either some providers need to update their profiles, or you need to hope some visitors meet your criteria.
