I had the same feeling come to mind too. I had an apt scheduled with her, but cancelled it once I read this. Sometimes the force tries to help you out, if you let it.  
BTW this thread combined with another thread could solve the mystery. I'd love to hear what everyone thinks. I'd really like to reschedule with her, but not worth rolling the dice. Her last review was Feb '15.

In fact I know a provider in just that situation right now.  By itself, I don't know why it should raise a red flag, especially if the provider has a solid review history.

is a red flag.  Pretty timely Star Wars reference, right?  But seriously it doesn't mean it's nefarious but me thinks you need to feel out the situation to determine yourself what's up or just ditch communicating with that provider.  Or perhaps consider giving us all more info as to the situation.

Also consider what that scheduler is holding on to as far as your personal info.  If you gave too much info up front consider getting a new phone number, job, car, and wife so they can't track you down.  ;-

I recently heard from what I believe is the same provider.  

I will admit that her abrupt departure several months ago and now reappearing does have me a little nervous about seeing her.

It might just be paranoia.

Everyone tells you what they either want you to hear (validating what you have shared) or what they want you to believe (oh no I'm legit but fucking you behind your back).  The only people you can trust are yourself and Wong...he tells it like it is bro.  ;-)
