Can anybody recommend a provider who will see two forty-something married men? Young and petite would be nice and she must be very open-minded.

Advanced Search is your friend. query for the "more than one guy at a time" and use the selection "really into it". There are actually quite a few providers who are into this. Haven't tried it myself but would like to someday. Have fun!
