name, age, ethnicity, and for three recent references of girls you have seen, AND their phone numbers.  If you don't have references, be prepared to give up your employment information.  Otherwise, you don't get it.  Its as simple as that.  

Search this board for a post I did a couple of months ago, suggesting the preferred language to use when texting the bookers.  Since that post, many newbies have PM'd me that they followed those suggestions and got approved.  If you don't give enough information and end up making extra work for the booker on your initial text, you often won't get a response at all.

Assuming LA go by what coeur said

If in the Bay, basically the same but 2 k-girl provider references is enough.

There are still a few that do not require references which makes it easier to get said references.

Stating that, there are also a couple of orgs that are more strict, meaning if your references are not well know orgs, and you do not have a good enough of a rapport 2 may not be enough.

While others say go for it, I never recommend giving out work or personal info
