FYI--just thought this might be helpful to some people on here.
There is a website through which you can arrange for anonymous testing for seven sexually transmitted diseases, including HIV, syphillis, hepatitis, etc. for a $229 fee. They have a bunch of locations in the Chicago area where you go to give urine/blood samples. You pay on their website and are assigned a PIN number and then go to one of the testing centers with nothing other than the PIN number, and later can get the results either by phoning in or by email, or online at the website with the PIN number. You do have to pay on the website with a credit card, but they don't require a name or address.

One of the cool things, besides the anonymity, is that you don't need a prior appointment, just walk in during their hours with your PIN number.
